    The paper should be word-processed, double-spaced, have one inch margins, and be in 12 point type. You will be graded for spelling, grammar, and coherent expression of ideas, in addition to the content of your paper.

    Follow the outline as closely as possible.  Use Roman numeral and letter headings [e.g., VI) c)] but do not repeat the questions.

    You should be able to complete the assignment in five-six pages.  If your paper is shorter, then you probably are not giving enough thought into the implications of the questions.  If your paper is much longer (i.e., more than eight pages) then you are probably repeating yourself too much. Take care to answer every question.  If you do not know the answer to a particular question (e.g., employment history), say so.  However you may create answers to certain identifying questions such as address and phone number or the names of minor characters.

    OUTLINE FOR BIO-PSYCHO-SOCIAL ASSESSMENT

    I) Identifying Information

    Name, address, telephone number.
    Date and place of birth.  Gender.
    How does the client identify him/herself ethnically?
    What are the names of those who reside with client?
    What is clients relationship with them?

    II) Referral Source

    Is the client a self-referral, or did someone else or an agency refer the person?
    What was the client told about your agency?
    Did the other agency contact you? If so, how did they present the persons problem?

    III) Clients Presenting Problem/Want/Need; Clients Initial Expectation of Agency

    From the clients perspective, what is the initial problem/want/need?
    In what way does the client anticipate the agency will be of help?

    IV) History of the Problem/Want/Need

    How did the problem start or form and what are the circumstances contributing to the problem?
    What previous efforts have been made to resolve it (if any)?
    Any previous contacts with other agencies?
    How has the problem affected other systems in the clients environment? (Family, school, employment, housing, etc.)
    How has the problem been affected by other systems in the clients environment? (Family, school, employment, housing, etc.)

    V) Other Significant Data

    Developmental history, including past and current significant health problems. (Physical and emotional.)
    Nature of current and past significant relationships and current social functioning.
    Education and/or employment history.
    History of substance abuse, if any.
    Hobbies.
    Any other significant data (e.g. trauma, poverty, military exposure)

    VI) Bio-psycho-social Assessment

    a) Current functioning.  Appearance of client; ability to communicate effectively; affect; roles; strengths; defense mechanisms; capability of functioning in pertinent life systems (family, job, community, school, etc.). Discuss the clients ability to cope with current problems, including how the client has handled stress in the past. What do you think is interfering with his/her functioning?

    b) External factors.  Discuss the strengths and limitations of pertinent systems in the clients environment.  Such as, availability of environmental supports (such as extended family, friends, religious groups) and agency or institutional resources (hospitals, day care, treatment programs.)

    c) Hope/Discomfort/Opportunity.  To what extent does client believe that change is possible; is motivated to action based on a felt discomfort with the present situation; and perceives that the opportunity for change exists, including the confidence that the clients environment will be receptive to change?

    d) Social workers understanding of clients problem/want/need from a bio-psycho-social perspective. (In your opinion, what do you think is the reason for the clients problem or situation? What do you think they want or need?)

    VII) Agreed-upon Goals and Plan

    What are the agreed-upon goals of the work with the client?
    Who is going to do what?
    When, where and how will the tasks be performed?
